Overall

Serious lack of communication among doctor/case worker/patient/spouse caregiver. Last week the case worker mentioned in passing that my 84 yo post lower leg amputation husband might be discharged Monday. I asked a nurse about it Friday and was told nothing was happening or we would be discussing it by now. Today (Monday) I had a call at home saying the ambulance transport was there to take my husband to the lower level care facility where he had been a pre-op patient. I was surprised and very unhappy because I don't think he was ready. Two staff people are required to move him from bed to chair to bathroom and he cannot stand alone holding onto a walker. No chance to talk to the doctor or anyone else. My husband-the-patient didn't know either until an aide started packing up his belongings. That was poorly done, too. His CPAP machine was packed sideways with the water reservoir still full and it leaked over everything, including two library books.

The nursing staff, OT and PT folks are excellent but the lack of communication shows little respect for the patient and his caregivers and I would not come here again if I had any choice.

Overall

Nurses and CNA's are fantastic! Case Management leaves a lot to be desired. You basically have to figure out your own discharge plan.
