Fat is located just beneath the skin. The lower trunk is where most women tend to accumulate fat – which is often resistant to exercise or weight loss. The layer of the tummy that provides the most strength and shape is the fascia and muscles. The stretching of these tissues during pregnancy cause the “pooching” that occurs afterward. A separation of the vertically paired abdominal muscles can occur in the midline creating a weak spot (“diastasis”). Even performing sit-ups or other exercises to strengthen the muscle are often not effective to eliminate the “pooch” – this is because the fascia and muscles have been irreversibly stretched and thinned.

If you can imagine a son or daughter, nephew, sibling, or other family member with a cleft lip or palate, it is easy to understand that this can be an emotionally and psychologically devastating occurrence. Fortunately, with a proper team approach to medical care, these babies will usually live normal and healthy lives. Ultrasound
Gynecomastia or male breast enlargement can be corrected through surgery. Depending on the degree of the enlargement and the characteristics of the enlarged tissue, either direct excision or liposuction is recommended. A newer technique, Ultrasound-Assisted Liposuction, is very effective in treating the frequently dense male breast tissue, while minimizing external scarring. A consultation with Dr. Ha will help determine which treatment will be the most appropriate.
Chemotherapy
From start to finish, breast reconstruction can take anywhere from 3 to 12 months depending on the type of surgeries required and any additional cancer treatment involved. Patients who undergo chemotherapy and/or radiation therapy may take longer to complete their reconstructions.

Dr. Ha received his undergraduate degree with honors from Brown University and his medical degree from Indiana University. He completed his general surgery, plastic surgery, and craniofacial fellowship training from UT Southwestern Medical Center.
Wound Care
Your wounds should be kept clean with either soap/water or with diluted hydrogen peroxide. This will reduce the amount of bacteria and germs and will help debride the scab that often forms. Tightly adherent scabs should not be forced loose, though loose scabs should be removed – these loose scabs can impair your wound healing.
Ear Surgery
The ear is a very specialized and unique structure of the body. The cartilage is the internal spongy material that provides shape and support to the external ear. The overlying skin is very thin and pliable. It tightly adheres to the underlying cartilage, which makes the hills and valleys of the ear very apparent. The external ear is very challenging to duplicate or manipulate surgically. Therefore, ear surgery usually requires the expertise of even the most specialized of all plastic surgeons.

Gangrene
One wrinkle filler that Dr. Ha DOES NOT inject is silicone. The FDA currently DOES NOT approve the practice of silicone injections in the U.S. Multiple studies show a significant incidence of complications with silicone injections. The silicone used in foreign countries or in the U.S. black market is often mixed with additives such as other types of oils, paraffin, and even peanut butter – these are often referred to as “sakurai” formulas and are even more dangerous. These incite a very aggressive inflammatory reaction by the tissues that can cause swelling, infection, redness, and pain. Hard knots can develop around any injected silicone, which are extremely difficult to remove. The silicone can also migrate through the skin causing extrusion and gangrene. Nerves and muscles can be permanently injured, and the resultant scarring is often irreversible or irreparable. There are cases after cases documented of patients undergoing multiple surgeries (15 to 30 or more) just to remove the material, even with “medical grade” silicone; these patients are scarred for life, permanently disfigured. Several deaths are reported every year that can be attributed to the complications arising from the injection of silicone.

Plastic Surgeons

Botox
The typical browlift candidates are those in their 40’s and older who have the beginning signs of brow descent and deeper forehead/frown wrinkles. They may complain of a progressively “tired” appearance of their eyes, but they may not exactly know why. These are often patients who have had Botox treatments in their 20’s and 30’s to help eliminate those finer wrinkles. Older patients may have so much extra upper eyelid skin, that it is obscuring their vision. These patients will often have very deep forehead wrinkles because of the constant sub-conscious contraction of the muscles to elevate the brow and the relieve the over-hanging upper eyelid skin.
Brow Lift
The goal of a brow-lift is not just elevation, but also, to rejuvenate and to re-shape the forehead and the upper eyelids. This procedure works well in combination with upper eyelid surgery to correct the “tired” eye appearance, because just removing extra skin of the upper eyelid is frequently not enough.
Eyelid Surgery
Asian eyelid surgery refers to a procedure to create a double eyelid fold that is typically absent in many Asians. The goal is to create an aesthetic eyelid crease that appears natural and balanced to a patients eye shape and ethnicity. There are 2 main techniques: Incisional and Suture. Patients may be candidates for one or the other depending on their eyelid anatomy, age, and expectations.
Liposuction
Fat transfer is a surgical procedure that aims to achieve these goals. Fat is harvested through liposuction with smaller cannulas to avoid excessive trauma to the fat itself. The donor site for fat is infiltrated with a wetting solution prior to the harvest to help facilitate the liposuction and reduce bleeding. The fat is then filtered and loaded into syringes. This fat is then re-injected into desired areas with small needles or cannulas. This technique is often referred to a fat micrografting, or layering of this fat in threads of small beads.
Breast Augmentation
After 14 years of restriction, silicone breast implants have once again been approved for primary breast augmentation (FDA Approval in December 2006). Silicone breast implants were removed from the market in 1992 after concerns regarding various health problems in women who received these implants, including pain, fatigue, arthritis, cancer, “silicone poisoning,” etc. In 1998, multiple class action and individual lawsuits eventually caused the bankruptcy of Dow Corning, the primary manufacturer of silicone implants.
Breast Reduction
The benefits from breast reduction surgery are the restoration of a proportionate and youthful breast contour. Many women who have natural C-cup (or larger) sized breasts can suffer from long-term effects of gravity. Those who have undergone large augmentations can also experience the same problems. Heavier breast tissue or larger implants causes increasing stress on the skin envelope of the breasts, potentially leading to stretching of the skin and descent of the breasts. Over time, the gravity of this weight overcomes the ability of the skin to support the breasts’ shape.

Rhinoplasty
Rhinoplasty can be performed either through an “Open” or “Closed” approach. This refers to whether an external incision is used or not. The open approach involves an incision through the columella (the column of tissue dividing the nostrils). This incision heals well and is usually imperceptible. The closed approach involves internal nostril incisions only. The open approach offers the benefit of improved visibility of the anatomy, and thus, easier correction of asymmetries and more accurate tip refinements. For most major re-shaping of the nose, Dr. Ha prefers the open technique because of superior control of the tissues and highly predictable and consistent results.
Breast Reconstruction
Breast reconstruction involves a series of life-changing surgeries and periods of recovery. There are many different options available to women today though careful consideration is given to the particular treatment course, depending on variety of factors. Generally speaking, reconstructive options are categorized into two main categories: autogenous (utilization of a patient’s own tissues) and alloplastic (implantation). There are pros and cons to each type, and some patients are not suitable candidates.
Plastic Surgery
Dr. Richard Ha is a board certified plastic surgeon with convenient office location in Plano, Texas. He specializes in pediatric plastic and craniofacial surgery and has expertise in cleft lip and palate, ear reconstruction and ear molding, craniosynostosis and other craniofacial anomalies, jaw distraction surgical correction for speech disorders, facial reanimation for paralysis, facial trauma reconstruction, microsurgery, tissue expansion and birthmarks. He is a member of the craniofacial team at the Pediatric Plastic Surgery Institute, and is also the president of the Dallas Plastic Surgery Institute.
Thigh Lift
A Thigh Lift (Thighplasty) is a powerful operation that can provide very dramatic improvements in the inner thigh contour. By removing the excessive skin and fat, a more pleasing and toned thigh contour can be achieved. There can be a number of different scar patterns depending on the technique used – this is largely determined by several factors.
Face Lift
The surgery itself is performed through well-hidden incisions in front of the ear and extending behind it into the hairline. Sometimes an additional incision is required just beneath the chin. The face and neck have several layers including the skin, fat, fascia (a strong layer of tissue covering the muscles), and muscle. Dr. Ha’s technique involves deep plane suspension (or SMAS facelift) – “tightening” of the fascia and/or skin, re-contouring the fat layer, and tightening the neck muscles. Previous notions of cutting out large amounts of excess skin has been supplanted by the newer concept of re-suspension, re-contouring, and preserving precious fat and skin. Dr. Ha believes that SMAS resuspension techniques have improved the longevity of face/neck lifting as well as providing more natural-appearing results. Also, he understands that deflation and loss of fat is a key component of facial aging; he focuses also on restoring facial volume through fat grafting or placement of implants in key areas.
Neck Lift
Facelift (or rhytidectomy) is a surgical procedure designed to rejuvenate and resuspend an aged and sagging face. It restores a youthful and sharp neck contour as well as improving the appearance of jowls. When Dr. Ha performs facelifts, a simultaneous neck lift is incorporated, in order to achieve optimal balance in the lower face and neck. Other facial rejuvenation procedures can be combined with a face/neck lift, including upper and lower eyelid lifts (blepharoplasty), browlifts, and facial fat injections.
Blepharoplasty
Blepharoplasty or eyelid lift is a procedure that can helps rejuvenate the periorbital area and eliminates that “tired” appearance. Small incisions are performed in the natural contours around the eyes, which in almost all cases leaves no visible scarring. The procedure has different goals depending on the anatomic location (upper or lower eyelid), the age of the patient, and the specific features of aging that are visible.
Abdominoplasty
Abdominoplasty, known more commonly as a “tummy tuck,” is a surgical procedure to remove excess skin and fat from the middle and lower abdomen and to tighten the muscles of the abdominal wall. The procedure can dramatically reduce the appearance of a protruding abdomen and restore its youthful contour. The stretch marks and dimpled skin that are typically located below the navel are eliminated. Frequently, a lengthy scar is required extending from hip to hip, but can be well hidden in most bikini lines.
Body Lift
Descriptions of procedures like breast lift (mastopexy), face/neck lift, arm lift (brachioplasty), and thighlift can be found elsewhere on this website. The classic procedure for post-bariatric body contouring is the Circumferential Abdominoplasty, or often known as the Lower Body Lift, Trunk Lift, or Extended Abdominoplasty. This procedure involves a traditional tummy tuck for the anterior abdomen, but also excision of excess fat/skin in the hip area extending into the back (above the buttock area). From posteriorly, the scar pattern resembles a gentle “V” above the buttock area. The scar is still concealed within the panty or underwear line. This procedure helps remove the redundant skin that is found in the abdomen as well as many of the excess “rolls” found in the sides and back. Depending on the technique and anatomy, the lateral thigh/buttock area can be lifted at the same time, to create a more balanced contour.
Breast Surgery
Otoplasty
The two main causes for this are a lack of the usual cartilage fold in the upper one third and/or an excess of cartilage in the bowl. Otoplasty is a procedure to reshape the deformed cartilage and excise any areas of excessive cartilage. This is done through a well hidden incision behind the ear. Otoplasty is best performed at younger ages when the cartilage is still very soft and malleable. As we age, all cartilage in our body (ribs, discs of the spine, etc.) becomes stiffer and more brittle; this makes re-shaping more difficult and less reliable. However, otoplasty can be performed with good results up to age 50 to 60.
Scar Revision
Once a wound has healed to a point where there is no open raw surface, it can be considered a “scar.” Scars are very dynamic entities; they are constantly changing, and can be influenced positively or negatively by external factors, which were discussed previously. There are many philosophies regarding scar “management.” Dr. Ha can recommend his approach to scar management during a consultation and help determine when or if a scar revision would be beneficial.
Lip Augmentation
Fat can be injected in varying volumes depending on the anatomic site. It is useful as an adjunct to most cosmetic procedures, providing finishing touches in contour. It is particularly useful in providing more fullness in the cheek area and filling in depressed naso-labial folds (deep smile lines) when facelifts are being performed. It can also be utilized as a volume expander, such as in buttock augmentation, or lip augmentation.
Body Contouring
Dr. Ha believes that the highest priority is a patient’s safety. Body contouring is ultimately elective and all appropriate precautions should be observed prior to proceeding with surgery. Multiple procedures at the same setting are usually possible and patients can benefit from shared recovery times and cost. However when the duration of surgery begins exceeding 6 to 8 hours, the risk of complications increases – including blood clots, respiratory failure, infection, post-op nausea and vomiting, etc. Dr. Ha does not exceed these operative times and in many cases will employ a co-surgeon to help maximize the number of procedures within this time limit. A comprehensive medical work-up is always performed and the treatment plan can change depending on a patient’s health appropriateness. Dr. Ha insists on performing these procedures in certified facilities with board-certified anesthesiologists.
Laser Resurfacing
Fine wrinkling can be improved with a good skin care regimen and minimizing sun exposure. More definitive treatments involve surgery by either chemical peeling or laser resurfacing. The most dramatic improvements for deep wrinkle correction occur through surgical face, neck, or brow lifting. However, deep wrinkling can also be treated by either reducing the muscular contraction or “filling” the crease.
Gynecomastia
Gynecomastia is the term given for excessive breast development in males. It has an incidence that is much higher than most people think. An estimated 30 to 40% of all males (and 60-70% in adolescent boys) have varying degrees of excessive breast development. Teenagers will often have a temporary course of enlargement that resolves spontaneously over a 2 to 3 year period. Adult males often exhibit permanent enlargement.

Bariatric Surgery
Dr. Ha can help patients determine their appropriate treatment plan for body contouring during a consultation. These are significant procedures that can be lengthy and can have serious complications if the appropriate precautions are not observed. Typically, body contouring procedures are best performed 18 to 24 months after the bariatric surgery has been completed. This is to ensure that physiologic changes and weight loss have been stabilized. It is important that a patient’s bariatric surgeon and internist are closely involved to optimize his/her health status prior to undergoing these extensive procedures.
Chin Augmentation
Chin augmentation refers to procedures to increase the vertical and/or horizontal projection of the chin. This can be accomplished through 2 different techniques, Chin Implant placement or Osseous Genioplasty. Dr. Ha performs both types of chin advancements and will help you determine which is most appropriate for you during a consultation.
Forehead Lift
A browlift or forehead lift is designed to elevate and reshape sagging brows as well as to correct the overactive muscles creating classic frown lines in the forehead and between the brows. This procedure is commonly performed with eyelid surgery to provide a balanced rejuvenation of the upper face. It can be performed through an open or endoscopic approach, depending on a patient’s tissue characteristics and the degree of lifting required. Dr. Ha can help custom tailor the appropriate procedure during a consultation.

Skin Cancer
Though Moh’s excision of skin cancers is typically performed in the face/neck/scalp/nose/lips and ears, it can be performed throughout the body. Dr. Ha has extensive experience in performing reconstructions of the defects left behind. He works closely with a number of Moh’s dermatologic surgeons to repair defects within 24 to 72 hours, and often times the same day. If you have had a biopsy confirming skin cancer, Dr. Ha offers consultations prior to your Moh’s excision so that all of the options can be reviewed. This can provide you with a framework of expectations and often give you peace of mind. He can also help refer you to the appropriate specialists for your skin cancer if they haven’t been arranged yet.

Acne Treatment
Chemical Peels
Resurfacing lasers (Erbium or CO2) essentially “burn” away the superficial layers of skin, creating a raw surface. By removing the aged and irregular outer layer, a new layer of skin can grow in. This “new” skin has fewer wrinkles, dark spots, and veins, and overall appears tighter, smoother, and refreshed. Darker skinned patients (including many Asians) are not good patients for this laser as it can cause white spots that are worse and more visible than the wrinkles that are being treated. Other complications can occur such as permanent redness or scarring from burns. Laser resurfacing and chemical peels work similarly. Lighter (more superficial treatments) can often be performed under local anesthesia; deeper treatments require general anesthesia. Despite what may be said, the “Day Spa” lasers are not equivalents substitute for the deeper resurfacing lasers. The latter provides a more significant improvement, but does require more recovery (up to 3 weeks).

After wrapping your head to reduce swelling, the first two days of recovery require special care and antibiotics before you return home. You’ll need to keep your head elevated for about 10 days. There is normally a follow up appointment after about 7 to 8 days, and another at around 12 days for removing stitches and scar management. You’re then ready to continue with skin care and have a check-in after about 3 months.
