Dental Examination
The severity of a toothache can range from chronic and mild to sharp and excruciating. It can be a dull ache or intense. The pain may be aggravated by chewing or by foods and liquids which are cold or hot. A thorough oral examination, proper tooth testing and evaluation, along with appropriate dental x-rays, can help determine the cause.
X-rays
During an apicoectomy, an incision is made in the gum tissue to expose the bone and surrounding inflamed tissue. The damaged tissue is removed along with the end of the root tip. A root-end filling is placed to prevent reinfection of the root and the gum is sutured. The bone naturally heals around the root over a period of months restoring full function. Careful follow-up with dental x-rays and clinical examination is necessary to confirm that the proper healing that is expected has actually taken place. Your surgeon will schedule follow-up visits at one week, one month, and possibly one year after an apicoectomy is performed.

An opening is made in the crown of the tooth. This allows access to the root canal system. It is very important to have a large enough opening to find all the canals inside a tooth. Anatomy inside the tooth is variable. Some teeth have just one canal like most upper front teeth. Premolars usually have 1 or 2. Molars or the back teeth typically have 3 or 4. Especially in back teeth, a surgical operating microscope with great illumination allows maximum visualization to help find these canals.

Dr. Martin grew up in the Austin area where he learned to love fishing and the outdoors. Following high school he served a two-year LDS mission in the Marshall Islands. After returning home he attended Austin Community College and then went on to graduate with a BS in Biology from Brigham Young University in Utah. While there, he met and married his wife, Amber. Dr. Martin attended the University of Texas Health Science Center at San Antonio, earning his DDS degree in 2010. He graduated as Valedictorian of his dental school class and stayed in San Antonio to complete a specialty graduate program in Endodontics in 2012. Dr. Martin achieved Board Certification with the American Board of Endodontists in June 2015, the pinnacle of education in Endodontics. The Martins have five young children and are excited to raise them in Central Texas.
Apicoectomy
Why would I need Endodontic Surgery? Generally, a root canal is all that is needed to save a tooth with an injured pulp from extraction. Occasionally, this non-surgical procedure (the root canal) will not be sufficient to heal the tooth and your Endodontist or general dentist will recommend endodontic surgery. The tip of the tooth root is called the apex; "ectomy" at the end of a word implies removing something. Therefore, an apicoectomy implies the surgical removal of the tip of the root of the problem tooth. A draining pimple in the gum above or below the tip of the root of a tooth reveals the infection from the dead nerve inside the tooth has spread beyond the tooth root and into the surrounding bone. This usually happens before root canal therapy is initiated and is resolved by removing the dead nerve. In rare situations, it will happen after a root canal and that suggests that an apicoectomy is needed. This surgical procedure can be used to locate root fractures or hidden canals that do not appear on x-rays but still manifest pain in the tooth. Damaged root surfaces or the surrounding bone may also be treated with this procedure.

Dental Implants
Root canal therapy is needed when the nerve of a tooth is affected by decay, infection or inflammation. This simple treatment can save your natural teeth and prevent the need of dental implants or bridges. At the center of your tooth is pulp. Infection of the pulp can be caused by trauma to the tooth, deep decay, cracks and chips, or repeated dental procedures. Typical symptoms of an inflamed nerve include sensitivity to hot, cold and chewing. If you experience any of these symptoms, your dentist may recommend non-surgical treatment from an endodontist to eliminate the diseased pulp. The injured pulp is removed and the root canal system is thoroughly cleaned and sealed. This therapy involves local anesthesia and may be completed in one or more visits depending on the treatment required. Success for this type of treatment occurs in more than 90% of cases. If your tooth is not amenable to endodontic treatment or the chance of success is unfavorable, you will be informed at the time of your consultation or when a complication becomes evident during or after treatment.

Dr. Bickel grew up in the small West Texas town of Ballinger. He graduated with a BS in Biology from Texas Tech University in 2000. Dr. Bickel then attended the University of Texas School of Dentistry in Houston, obtaining his DDS degree. Following dental school, Dr. Bickel joined the US Army, and completed an Advanced Education in General Dentistry residency at Fort Sill, Oklahoma. He was then stationed in Darmstadt, Germany for 3 years. Dr. Bickel was selected to the Endodontic specialty program at Fort Bragg, NC. Following the completion of the endodontic residency, Dr. Bickel spent 3 years in beautiful Savannah, Georgia followed by 4 years at Fort Hood, TX. While at Fort Hood, Dr. Bickel served as an Assistant Professor for the Graduate Dental Program of the Uniformed Services University. He is finishing out his military career in the Texas National Guard. He and his family have lived in Georgetown since 2013 and absolutely love the community. Dr. Bickel is married to the love of his life, Megan, and they have two sons and a daughter.
