This is verification that I have brought my child for dental examination and treatment. I hereby consent to have all doctors and associates of KinderSmiles perform dental examinations, cleanings, fluoride treatments, dental restorations [including, but not limited to, tooth colored fillings, silver fillings, and stainless steel crowns, dental x-rays(when necessary), dental extractions and any other procedures deemed necessary oradvisable as a corollary to the planned procedures. I further agree to theuse of localanesthetic agents, oral and intra-venous sedation agents depending upon the doctor(s)involved in my care.

Just like with oral sedation, patients should avoid food and drink for several hours beforehand and adult patients should arrange for a ride home afterward.

If their braces got broken instead, try to reattach the loose wire with a cotton swab or eraser. If you can’t, be sure to cover it with orthodontic wax. Make sure to also recover any broken pieces of the appliance and bring them with you to our office.
