Medical Weight Loss
Dr. Vierra and Dr. Chang, with the help of the dedicated PA, Deirdre Hamilton, perform gastric bypass, sleeve gastrectomy, and laparoscopic adjustable band surgery and provide continuous coverage for these patients within our group. As of January 2015 we have performed >2000 weight loss operations at Community Hospital and have some of the best safety experience anywhere.
Primary Care
In general we ask that patients be referred by a primary care physician. If you do not have one, we can assist you with recommendations for a local physician.
Emergency Care
Should you have an episode of pain that lasts longer than 8-12 hours, or jaundice (your eyes and skin will turn yellow and you may notice that your urine turns dark) then you should contact the office if it is during the day, or going to the emergency room at Community Hospital. It may be safest for you to be hospitalized and your operation performed sooner than scheduled.
Electrocardiogram
On the day of your operation an IV will be placed and you will be given sedation if you need it. You will be taken to the operating room where EKG leads will be placed on your chest and all monitors will be put in place. From the time you leave the preop holding area until we are ready to begin the operation takes about 20 to 30 minutes.

Lane Verlenden, M.D. is a graduate of the University of North Carolina and Harvard Medical School and completed his residency in internal medicine at UCSF in 1974 and his residency in surgery at UC Davis in 1984. He is Board Certified in General Surgery and fellowship trained in vascular surgery. He has been with the practice since 1984 and he is a Fellow of the American College of Surgeons.

Kidney Stones
Breaking up the stones with shock waves as is done with kidney stones (lithotripsy) has largely disappeared due to concerns about effectiveness, recurrence of stones, and issues of cost. Medication to dissolve the stones can be taken by mouth and may dissolve a small minority of patients’ stones, but recurrence of the stones is common when the medication is stopped and the medication itself is costly.
Colon Cancer
Our group cares for the entire range of surgical conditions of the colon (large intestine). Unfortunately colon cancer continues to be one of the most common problems we treat. Our goal is always cure and we can often accomplish successful removal of the colon tumor with its adjacent lymph nodes using laparoscopic (minimally invasive) techniques, although occasional patients are still best treated with open operation.

While most thyroid conditions do not require surgery, thyroid cancer, large goiters (enlarged thyroid gland) and certain instances of hyperthyroidism (overactive thyroid) are best treated by surgery. We work closely with endocrinologists in deciding who will be best treated with surgery and carefully tailor each operation to the individual patient.

Mark Vierra, M.D. is a graduate of the University of California San Diego and Harvard Medical School and completed his residency in surgery at Stanford University in 1990. He joined the Stanford University faculty in 1990, and practiced there until 2001, when he joined MCSA. He is Board Certified in General Surgery and a Fellow of the American College of Surgeons. Wound Care
Wound care: When you are discharged you will have tape strips across the wounds. You may shower on the first day after the operation and there is no need to keep the tape strips dry. Simply pat them dry when you are done. They will peel off on their own in 710 days. There are no sutures to be removed.

Jon Benner, M.D. is a graduate of Yale University and USC School of Medicine and completed his residency in general and vascular surgery at the University of Michigan in 1980. He is Board Certified in General Surgery and is a Fellow of the American College of Surgeons. He has been practicing general and vascular surgery in Monterey since 1980.
Laparoscopy
The vast number of cholecystectomies done in the US today are done by laparoscopy, which is a technique in which the abdomen is entered with small, narrow tubes, and the abdomen is filled with carbon dioxide. Long, slender instruments are then inserted through the tubes, and the procedure is watched on a video screen. The advantage is that because the incisions in the abdominal wall are so small, there is very little pain, and recovery is quick.

Breast Surgery
Breast surgery is evolving apace with the rapidly advancing approaches to breast cancer, emphasizing breast conservation, cosmetic excellence and improved long-term survival. We are committed to a multi-modality team approach and work very closely with the Breast Care Center of Community Hospital of the Monterey Peninsula as well as medical oncologists, radiation oncologists and pathologists here and at university centers to tailor each patient’s care to her needs.
Laparoscopic Surgery
We perform only the Roux Y Gastric Bypass, Lap Band procedures, and sleeve gastrectomy, and of course revisional surgery. We can perform these by open or laparoscopic surgery, but in practice > 98% of our operations are performed by laparoscopy. Dr. Vierra was among the first surgeons in the world to perform laparoscopic weight loss surgery, and in his practice conversion to laparotomy occurs in < 1/500 cases.
Bariatric Surgery
Community Hospital of the Monterey Peninsula, along with Monterey County Surgical Associates and Dr. Steven Chang and Dr. Mark Vierra are all recognized as being a part of the Bariatric Surgery Center of Excellence in 2006 — one of the first on the West Coast. Our program was established by Dr. Mark Vierra, MD, FACS, who started the Obesity Surgery program at Stanford in 1991 and directed the program there until he moved to Monterey in September of 2001. With over 20 years of experience in bariatric surgery, Dr. Vierra is among the most experienced bariatric surgeons in the US.

Ulcer
Medications: Avoid tablets of Aspirin, Advil, Motrin, Naprosyn, Indocin, Fosamax, tetracycline, Vitamin C and all other similar medications that are known to cause ulcers for at least 2, and preferably for several weeks. Do not take them until you are swallowing solids without difficulty, and take them with food. If these get hung up at the area of the wrap they can rapidly cause a dangerous ulcer. Elixers of these medications are safe, however. Tylenol is safe in pill form, but comes in an elixer, as well.
