One other item we want to get to right now and that is although it is still in experimental stages; researchers say a new melanoma drug could see a sizeable shift in cancer treatment. Dermatologist’s Jeanine Downie joins us today about the be well health report. Tell us about this drug – hi Chris so this is called Ipi Limiabob and it is a molecular antibody that is a mouth full it is actually a drug made from living cells, so think about it as a protein that attaches to white blood cells and help to fight off the cancer that was. The only way it works is in about 20 to 30 percent of people but about 70,000 people have melanomas and about 9000 die from melanomas here in the United States. So we are talking about 20 to 25 percent of those 9000 people, and it just clears it out and we are not sure why it works on some people and why it does not work on some people. Just think about it like hyper stimulating the immune system so what is happening is the side effects form the auto immune diseases are triggered like rheumatoid arthritis, it is not the usual sickness anything like that – but it is a huge breakthrough. I mean I am really surprised can’t tell a difference. They are great as a source of fiber and everything else! Thing of regular you could be if you had these every day. A cup of quinoa is packed with protein and a cup of cooked quinoa has 8g of protein whereas your brown rice will only give you 5 and there is a study that looked at appetite and they found that quinoa provided more satiety which is a fancy way of saying staying power. The strategy in terms of success for me getting my own space as soon as possible came from a couple of things. 1 my grandfather is a dentist he god ret his soul he has passed now back in the roaring days of Harlem he owned his own space in Harlem and he thrived there and he loved the fact that he owned his own space. My mother retired about a year and a half ago from pediatrics now and she owned her own space kind of like a suburban dentist office – the office was on the side of our home growing up so I saw those 2 role models now – what myself and my own private practice I found early on that the gentleman that I was a hiring office space with was a neurologist right here only a block away and I felt that the more kind of Mercedes and BMWs that I had in the parking lot the higher my rent would get after a while – it got to be this kind of competition thing with him like he wondered how this young dermatologist was attracting all these patients, and he felt like he could just continue to charge m more and more. So what I did was – I was pregnant with my daughter at the time and I just said I can’t take this anymore this is not fai – this is not good – he wanted a loose contact and I wanted something that was firm from year to year but he wanted it loose so he could continue to charge me more –and I went out and I looked around and I decided that I was going to purchase this 1870 Victorian we are in right now, it needed a lot of work and I just decided that I was going to be the woman for the job – so I bought the building back in 2003, we closed – it was supposed to be a 4 month renovation – it wound up taking 2 years – the town made me put an elevator in it still is in the Mont Clair historic registry because I kept it historically accurate and it has been one of the best investments and it is cheaper the mortgage here than the rent I was paying on the other place – now that I am well established in my own space I can just tell you it is nice to have the building and have ti be your building – simple things like the temperature of the boiling, what the climate in here is like. A lot of my patients in my old private practice were getting hot and almost fainting during biopsy’s and procedures, because they were nervous to being with and a lot of the women that were working in the other office liked the temperature elevated – I like it cool and it is better for patients, they get nervous during procedures if the temperature is cool. So simple things like that really make a difference and it seemed like so nothing but the extra 5 degrees every day that you are either arguing about or parking spaces you don’t have to argue over parking spaces for your staff and for someone else’s staff – it is just simple things like that I would never want to go back to being a tenant again because you can kind of lord over your destiny a little bit more and I prefer that.

Genito Urinary Syndrome of Menopause (GSM) is defined as a collection of symptoms and signs associated with the decrease in estrogen involving changes in the labia majora and labia minora, clitoris, vagina and bladder. Some of the symptoms are dryness, burning, itching, painful sex and post coital bleeding. These issues face a great percentage of women and this number continues to grow. This not only applies to women who are Perio and post-menopausal, but young women on medication, women who have had hysterectomies, or simply dealing with the stress of life. We have democrat republican, democrat, republican, and democrat, republican. So we are fair and balanced about the issues that we are to speak on. Carmen I will start with you – what is the most important thing in your household? I like rule of law and I don’t I am not a fan of socialism or wealth distribution my husband works very hard and we like to plan for the future and I don’t want him working so our neighbors can go to college for free! A lot of people feel the same way – so what does he do? He works in health care. Ok and what do you do? I am a stay at home mom, so I have been at Costco all last week stocking out on stuff. You are worried about it – I am and I just would rather on the side of caution, than be disappointed with bombshells. Now Dr. Jeanine is back here you know this panel we have had them on before! They are wonderful and we love these segments because we are – this is what we do on the weekends if you want to be a fly on the wall amongst women we talk about issues and we talk about our kids and so Dr. Downie you are a dermatologist board certified – in New Jersey congratulations that is not easy – top of your class. How many kids do you have? One child she is a teenager – how is that going she is great actually she is lovely and I am very proud as a mom – it is the best thing I have ever done. So what is imprint t you – education is important to me, access to education I don’t necessarily want free education, like you just said I want access to education I think hat there should be good public education through high school. Health care is absolutely critical someone on this panel just recovered from cancer. Curing cancer access to health care, aces to proper prescription drugs, access to the things that we need – insulin shod not be as expensive as it is, that is ridiculous. I agree. And then in terms of gun control – I am worried about the safety of our children and ourselves with people walking around with guns all the time – as a democrat what do you think about Bernie sanders plan? As a doctor a health care plan and free college for everyone how are we going to pay for that? So I am a Biden girl or a Bloomberg girl. I am not a Bernie girl at all. That being said I want to defeat trump so I would be a Bernie girl if I had to be a Bernie girl I am not for Medicare for all in all – I don’t think it is going to work. Skin cancer is the most common form of cancer with more than 3.5 million people nationwide diagnosed with it annually. Melanoma is the most deadly form of skin cancer an estimate 68000 new cases of melanoma diagnosed in the US in 2010 – it account for only 3 percent of all skin cancers but causes 80 percent of debts. Now there is some good news in treatment and there is a new drug just approved by the FDA and is ready for use. Here to tell us about this is dermatologist Dr Jeanine Downie – Dr. Downie welcome. Hi Tanya go to be back – good to see you so this is good news there is a successfully new drug out there - yes it is called epi limumob it is a monocular antibody and the trade name is Eruvoid it is marketed by restil Myers web and was approved on the 2th of march very recently and how does it improve the current treatment for stage 3 and stage 4 melanoma? Well what it does it blocks not to get to technical but sitotozix tee lymphocytes – it is so exciting because nothing has worked for stage 3 and 4 melanoma patients at all. They have had high dose introluken and for 35 years it sometimes works, may times doesn’t and these patients have nothing and now what it’s doing its activating these psyto toxic T cells, and they are acutely helping the immune system target recognize and block melanoma. So it has a lot of success so far, and is your boy working for everyone or is there some patients it doesn’t work for? Great question – it doesn’t work for everyone and we are not sure why but for the people that it works for it actually hyper stimulates the immune system and it allows the body’s own immune system to go out and target and recognize these cancerous cells and kill them so it worked really well for the percentage of the population that it does work for. The clinical trial worked for 677 people in it so it is a large clinical trial and this is a huge thing for melanoma suffers because it is crazy these days. Absolutely – used to be in 1930 that 1 in 3500 people would get melanoma, now it is down to 1 in 53 according to the skin cancer foundation. It is – that is such a scary increase and what else accounts for that – more people who are going out tanning is it tanning bed? If you have an early exposure to tanning beds as a teen it increases your risk of melanoma by 75 percent it is ridiculous. So tanning beds are 5 to 6 billion dollar industry and a lot of young women will wind up unfortunately late with melanoma there is tannin and increased leisure time and we see white men over the age of 50 – it is scary and it is something you see in women and men, I think I told you when we were talking g before it is the 5th most common cancer for when who are all, and the 6th most common cancer for women. It seems that early childhood exposure to sun to extreme burns – blistering sun burns; both of us have young kids. One blistering sun burn more than doubles your risk of getting melanoma. I love it and the next one is an app that I use – for my own diet tracking. Me too! My fitness pal – you use it as well? I would give you a fit bump but we are too far away. I don’t want to hurt the monitor either – so tell the folk a bit about this. So they have scores of cardio, strength exercises they have over 2 million different foods both prepackaged foods and otherwise foods in their calorie counters so you can figure out what you are doing per day. Track your calories and track you fitness put it all in there and figure out like you said before okay, I really want this alcoholic beverage or for me I really want this piece of chocolate or cheesecake right therefore I am going to go out and I am going to exercise more so I can have my chocolate you know – Il tell you what I learnt by using this I learnt where my colors were going everyday which I found really useful. I thought wow I can’t drink that much milk or I can’t have that much cereal that is – it was really informative right, so next is my quick coach – ok so my quick coach is great and the bottom line is if we could stop everybody in the world from smoking this would be a wonderful thing. So are you worried? Actually good morning Jedadiah – thanks for having me on. I am concerned. There is a hidden pandemic out there of untreated conditions so so many patients that can’t go to the doctor our goal should be to keep people out of the ER and out of urgent care centers and there are all these different conditions that are treatable and preventable in the doctor’s office and without us – there is an increase in heart attacks, an increase in strokes me particularly there is an increase in skin cancers aneurisms, kidney failure and so many of these things that are preventable and so there is downstream health effects of keeping doctors’ offices closed. And I as a board certified dermatologist am very concerned about that – a hundred percent. Dr. McCarty I want to ask you about this tweet from governor Quo Mo on May 13th it read – reminder doctors’ offices are essential business and continue to be open to patients across New York state- check with your doctor or hours and protocols I live in New York and that has been untrue! I could not get my baby in to a dermatologist in person for a rash that was concerning – I myself had a non Covid related issue long after I was out and tested negative for Covid after tested positive – I had completely recovered an I could not get in. so he is not being truthful – so what do e do about that, this misinformation is out there and doctors’ offices are just not accessible to people the way it should be. Well Jed good morning turns out there is a big backlog of cases and it’s from the time we had to shut down so every doctor’s office is different and I think most doctors are ready to get back to work and most offices should be able to open with the exception of maybe some pockets to the country you know Montgomery Alabama has had a recent outbreak, but for most of the country some of the early draconian moves that we did –make sense then but right now we need to evolve our strategy. Now to Vitiligo a skin condition you have heard of but may not know much about! Micheal Jackson claimed he suffered from it to explain why his skin colour became lighter over the years – well 23 year old Marcel began to show signs at the age of 5 and for the past 7 years her skin has gone completely white and now she is here along with Dr. Jeanine Downie to talk about this – good morning to oh of you – let’s talk first of all. What happened with you say it’s stated with your arms and legs when you were young then at age 12 what happen? Well I got a post on my arm at the age of 5 that is the first sign of it and then one on my forehead and from the ages of 5 to 12 it slowly was going through my arms and legs and on my torso and then by the time I reached 12 years old it just started spreading very rapidly on to my face neck. During those years when you are pre-teen a teenager all of that – ta was when it was all changing? So that is very hard to deal with to explain to people. Did people treat you badly as a result? I had a really strong close knit group of friends my mom was very strong as well she really pushed me out there to do a lot of different activities but then you do get the comments that you know when you don’t have people around to protect you and you are not too sure how to respond to it - you do also hear things that people have said. I have been called spot, I have been called Dalmatian – I have had people say during a sleepover don’t sleep in the same room or you will catch what she has and you will die. Okay so how do people get this condition? Well we are not sure exactly – Vitiligo affects 3 percent of the population and you see it in all races and ethnicities – obviously it is more noticeable with people who have darker complexions – bottom line – we think it is genetic, most people have either primary or secondary relativeness 25 to 30 percent of people who have Vitiligo have it in the family as well. You do have people in your family? I don’t have any blood relatives who have it – as far back as we can think however I have family by marriage extended family my dad’s step brother has it basically the way I had it – half black half white all over and then another distant family amber has on her face. You mentioned auto immune issues – besides the idea that the skin would change color what are other negative effects? Example some people with thyroid disease may also have Vitiligo – some people who have diabetes may also have Vitiligo – people how have anemia would also have Vitiligo – but is there any other secondary effect beside your skin changing colour? Well you are more sensitive to the sun – so people who have Vitiligo tend to blister and burn more and are more susceptible to skin cancer. Ok s that is a big issue but anything less besides that? Well as we age – like Richard said thank you very much Richard again – the bottom line is if you have amole that you have had for a while and it begins to change, it beings to get different colors in it, it raises up it looks bigger anything about it – changing some moles actually start to hurt they start to bled and you get a new pimple on your face that doesn’t heal like atypical pimple does those are signs that something is changing and that you need to go see a board certified dermatologist. So we get a lot of moles as we age right absolutely and I mean the moles sometimes come in parts of your bod of your women – like where your bra is I mean what is that there is no sun damage there! How the heck did they get there? So that is family history – that is becoming more mature, right? I don’t like the mature bit – I know! The bottom line is it is a bit of sun damage a well. Everyone needs to go see aboard curtsied dermatologist once a year to get a full body screen that is what we do a head to toe we look from your scalp down to between your toes and make sure you have no abnormal moles and that is once a year – and if you have a history of skin cancer then you need to go more often like Richard needs to go every 3 months and also probably because he is a red head and a fair skin. Well we want to looks young and as healthy as we can for as long a s we can – some of the damage already done how do we continue to protect our skin and care for it? Well the bottom line is there are some anti-ageing ingredients tat work better than others. 6 percent of American doctors are black and in some specialties it is even worse – like dermatology just three percent of dermatologists are black, the next generation of medical students are looking o change that. CNBC Valerie Castro explains – 4th year med student Victoria Humphrey is following her dream studying to become a dermatologist at the university of Pittsburg school of medicine. I would love to focus on serving underserved populations and skin of colour. A childhood illness is part of the reason she is pursing dermatology. I actually had Kawasaki disease, and I received a delayed diagnosis because the rash did not look as quite the same as on my darker skin tone – Humphrey says that experience ignited her interest in the field – but she quickly noticed ad lack of diversity during her studies – medicine itself is not the most diverse field, and being a black woman in medicine being under represented I knew that it was an area that I can make an impact. Some patients would clap and smell and they would say I can’t believe it I am seeing a doctor who is black or brown a skin of colour! Doctor Elena James is Humphreys mentor – she is also a practicing dermatologist at eh university’s medical center. Patients are really looking for someone who is an expert in the field because there are a lot of differences the way skin disease presents. Doctor James saying knowing those differences and being a doctor of colour can dramatically impact a diagnosis and treatment plan. Three have been studies to look at risk affording care and some of these studies have shown that there is a benefit of a patient staying a provider that is culturally race similar to them so they are so much that is packed into being a black person in America – we know that we are judged simply by the colour difference but of our skin and the treatment we receive maybe based on that as well – Doctor James remains positive about the future of dermatology and students like victoria eager to not just make a difference but be the face of change. There is a new energy that is here to put racism on the front line and coming up with strategic ways of overcoming it – I see Victoria standing up and making sure that her voice is heard – it is such a wonderful thing to be able to serve populations one that looks like you and 2 hot really need our help and our care the most – dermatology maybe one of the least diverse medical specialties but overall diversity in medicine could be on the rise. Some medical schools are reporting an increase of more than 40 percent in African American applicants sighting the pandemic and recent social justice movements as the motivating factors. Yeah do you even know what a loofa was? Vaguely – so a loofa harbors bacteria fungus and people use them all over – we don’t recommend them and people use them all over. Dermatologist says so just get rid of the loofa – ok dr Downie next one try to sleep n your back why is that imprint? As you sleep on your face you crush your face and you get more wrinkle sand you shod really sleep with a satin pillow case if you can because your hair is not stripped than and your skin is not stripped from your ageing products at night, if you are old like I am and have to use anti-ageing products – you are doing ok yeah you are looking amazing. But you say you can actually tell by looking at some on which side they sleep on – okay do you sleep on that side of your face Jenna ok, that is horrible – do you want me to do it to you too? No! That is awful that means I have more wrinkles on that side. For accuracy of it – the worst part of the segment is that you have 2 great dermatologists close to you analyzing your face, and that is not fair! It’s because of my mouth wrinkles you could tell – and the cheek wrinkles too. You look great and good and you are very pretty. Nice catch - alright Dr. Grossman how about mixing up your exercise routine? For example – don’t run every day do different things on different days. Very important people who do repetitive exercises in the same way it shows running especially, if you run every single day you lose fat in the lower part of the face and as you age it really shows up as sagging an wrinkles. Swimmers also have to be careful. If you use goggles you get some trauma around the eyes you need to mix it up and i like the larger goggles that are more like a makes but it is important to exercise. I am not saying don’t exercise – so important for your skin but you should do moderate exercises like 20 – 30 mins a day – t increase your glow factor, it boosts your circulation, it tones your muscles I exercise 7 days a week it is critical. And we can tell! You do! You look great and your skin looks beautiful and now let’s move on to the eyes which are the most sensitive of our face. Ok so im going to take this on I have
