In either case when the surface of the joint is worn away, at a certain point in time walking and activities of daily living become very difficult. Smoking Cessation
Do not drink, eat, or smoke after midnight prior to your surgery day. Smoking cessation at least 2-3 days prior to general anesthesia will significantly decrease the chance of postoperative pulmonary complications.

MRI
MRI stands for magnetic resonance imaging. It is a non-invasive technique for imaging the spine that involves rotating a magnet around the body and exciting its hydrogen atoms. A scanner is then utilized to detect the energy emitted by the excited atoms. Because the human body is composed primarily of water, which is two parts hydrogen, an MRI provides exceptional detail of the spine's anatomy and is the single most useful test available for diagnosing spinal disorders.

A CAT (computed axial tomography) scan, also known as a CT (computed tomography) scan, is a painless imaging technique that utilizes a computer to produce detailed three-dimensional images of a body from a collation of cross-sectional x-rays taken along an axis. Of all the imaging techniques that are currently available, the CAT scan is best able to produce images of bone and metal devices.

Dr. Shonuga specializes in Orthopaedic Hand & Upper Extremity Surgery at NoVa Orthopaedic & Spine Care, a division of the Centers for Advanced Orthopaedics. He obtained his medical degree from Weill Cornell Medical College and graduated with honors after completing a research fellowship at the Hospital for Special Surgery in New York City. As an undergraduate, he attended Rutgers University and graduated with a Bachelor’s degree in Biological Science. He completed general surgery internship and orthopaedic surgery residency training at Rutgers - Robert Wood Johnson University Hospital. As a chief resident, he was presented with the resident of the year award and was nominated to participate in the American Orthopaedic Association’s Emerging Leaders Program. After residency, he completed fellowship training in hand and upper extremity surgery at the Rothman Institute/Thomas Jefferson University Hospital in Philadelphia. Throughout his training, Dr. Shonuga has published multiple journal articles and book chapters on orthopaedic related conditions. He utilizes current concepts in the field to deliver evidence based care in a compassionate manner.

Minimal Access Spinal Technologies have been developed out of advances, in the field of orthopedic minimal access surgeries over the past two decades. Many surgical sub-specialties use fiberoptic video cameras, endoscopes, catheters, and specially designed tools to assist in performing surgery with much smaller incisions. Today, some of these procedures have become the standard of care in orthopedics, cardiology, and gastro-intestinal medicine. Most people are familiar with this type of technology in the form of "arthroscopic" surgery, which allows orthopedic surgeons to look inside joints like the knee and operate through very small incisions. Many people have also become familiar with this type of technology through cardiac catheterizations that are used to treat coronary artery disease, and endoscopies that are used for the diagnosis and treatment of problems of the digestive tract.

If disc or bone material pushes into or impinges on a nearby nerve root and/or the spinal cord, it may result in pain, numbness, weakness, muscle spasms and loss of coordination, both at the site of the damage and elsewhere in the body. Sciatica - often described as pain that begins in the hip and buttocks and continues all the way down the leg - is one example of a radicular symptom that may be caused by damage or deterioration in the lumbar spine.

Pinched Nerve
Cubital tunnel syndrome is a pinched nerve at the elbow commonly known as the "funny bone". This might be caused by trauma or repetitive use of the elbow and may be caused by continuous use of the elbow in a flexed position. This causes the nerve to become stretched and irritated as opposed to when the arm is extended and the nerve is in a relaxed position. The diagnosis can be confirmed with electrodiagnostic testing including nerve conduction velocity and the electromyogram. Nerve conduction velocity studies, the speed of the nerve across the elbow, will be slowed when there is nerve compression and electromyogram studies, the innervation of the muscles, might be affected by the pinched nerve.

Joint Replacement
Surgery -- as the last resort, when conservative treatment has failed -- consists of a joint replacement using the patient's normal body tissues and involves excising the arthritic bone and replacing it with a tendon taken from the wrist which is rolled up into a ball and used as a spacer and a portion of it is used to reconstruct the ligament. This is done through a small incision at the base of the thumb and a smaller incision at the base of the wrist to harvest the tendon used for the graft. It is an outpatient procedure performed under axillary block where only the arm goes to sleep. The patient is immobilized in a splint for two weeks, then a thumb spica cast for two weeks and then uses a removable custom made splint for two months while they are undergoing therapy for their thumb.

Knee Replacement
In many cases, however, despite the above non-surgical treatments, functional limitations persist. Most people who are considering knee replacement are limited to walking less than three to six blocks, or less than 15 to 20 minutes. They have difficult time getting up out of a chair. They have rest pain. They are taking anti-inflammatory medication and/or pain medicine on a regular basis and the pain is generally progressive.

Only recently, however, have surgeons begun to fully understand the nature of ACL injuries in skeletally immature athletes. Specialized study of ligament injuries in children is necessary because of the unique biology of growing bones and ligaments. Longitudinal growth in the child takes place at the ends of the long bones, with particularly rapid growth occurring around the knee at the end of the femur (thigh bone) and top of the tibia (shin bone). Growth plates (where growth occurs) are comprised of a sliver of specialized cartilage through a cross section of these bones near each joint. The bones literally increase in length and grow away from these areas. ACL surgery presents unique issues in adolescent or skeletally immature athletes because the reconstructive surgery involves drilling tunnels across the growth plates and placing a graft (replacement ligament) into the knee joint through these tunnels.

Dr Lotfi utilizes plastic surgery/cosmetic techniques for closure of your incision. You may have absorbable sutures. Skin may be closed with Dermabond (medical-grade Crazy Glue) which has a crusty consistency. The incision can be washed with soap and water. If used, Non-absorbable sutures will be removed usually 2 weeks after the surgery on your first post-op visit.

Dr. Richard Layfield is a native of Silver Spring, Maryland and specializes in Sports Medicine at the NoVa Orthopedic & Spine Care division in Woodbridge, Virginia. He gained experience working closely with sports teams like the Minnesota Vikings, Minnesota Twins, Minnesota Timberwolves and the Minnesota Wild while completing his fellowship training in Sports Medicine at the Orthopaedic Center in Minneapolis, Minnesota. Dr. Layfield continues his commitment to athletic care in Prince William County by serving as head team physician for the Prince William Courage Soccer Club. He is also an orthopaedic and sports medicine consultant for many of the local high schools in Prince William County, including Potomac, C.D. Hylton, Garfield, Forest Park, Freedom, Woodbridge and Osbourn Park.

Before surgery is considered a trial of at least six months of conservative treatment is indicated and may consist of a properly placed forearm brace and modification of elbow activities, anti-inflammatory medication and physical therapy. If the above treatment is not helpful, a cortisone injection can be beneficial but no more than three injections are recommended in any one location in a year.

After attending the University of Virginia, he completed his medical training at Temple University School of Podiatric Medicine. He then finished his residency training at Inova Fairfax Medical Campus, where he served as chief resident. Dr. Boparai brings extensive expertise in all areas of foot and ankle care with a special interest in sports medicine, flatfoot reconstruction, and minimally invasive surgery. He has authored numerous publications and posters in peer-reviewed journals and national conferences. In his spare time, he enjoys exploring restaurants, Washington DC sports, and spending time with his wife, and two golden retrievers.
