Cntry Villa Lynwood Healthcare Center

Diabetic patient had low count when I arrived. She was non responsive and was in a cold sweat. I requested to speak to the Nurse In Carge. I asked for info on the patients conditon she claimed she was OK and Refused to check her vitals and FALSLY claimed to have done them 10 min. before I arrived. After I called 911 the paramedics arrived and the head nurse would not release or supposedly lost the patients chart. The care givers appologized for the situation but by law they can not do anything or interfere. From what I last heard that nurse was still employed by them.

Patient was taken to better facitlity in Maywood.
