        After many years of loyal service to them, they randomly started discreetly billing my insurance under a line item code of D1330 which gets billed to me. The name of this code is "Oral Hygiene Instructions". This is IN ADDITION to my normal oral examination costs. These charges were not always on my bill, they recently started appearing. I called the main office to have these removed and they had absolutely no explanation as to why they are there or even knew what they were. I won't be paying this charge which is $76 per visit. This should be illegal.
