I am so relieved to not be suffering any more! After 10 years of mis-management, having a correct diagnosis and treatment for vestibular migraines is a blessing. Ashly’s nursing journey began in 2006, when she graduated from Ivy Tech Community College with a certificate in licensed practical nursing (LPN). Her first nursing position was in long-term care, in the same facility in which her grandfather was a resident. Her grandfather, who passed away in 2007 due to complications of Alzheimer’s disease, was the catalyst for her passion for neurology. Since then, she has practiced in a variety of fields, in both inpatient and outpatient settings. Neurology continues to be her preferred specialty.

Once you have checked in, Missy will be the one calling you back to check your heart rate and blood pressure and updating your chart. She administers Tysabri, an intravenous medication for treatment of multiple sclerosis (MS). She has experience with hospital, pediatric and geriatric patient care as well administrative nursing. Missy works with Dr. Sanders to ensure that patient care is both accurate as well as compassionate. She enjoys educating patients on their care and treatment plans.

Diagnosis is suspected based on audiogram revealing unilateral (one-side) hearing loss, specifically sensorineural hearing loss, and vestibular function testing abnormalities. Vestibular function testing abnormalities may include caloric weakness on the same side as the hearing loss, spontaneous nystagmus typically in the direction opposite the affected side, reduced VEMP respose on the affected side, and rotary chair abnormalities including reduced VOR gain, VOR asymmetry and phase lead. MRI of the brain or IACs (Internal Auditory Canals) with contrast is the best way to confirm the presence of an acoustic neuroma.
Computed Tomography
To the right, a CT scan of the temporal bones (where the inner ear structures lie) reveals lack of a 'roof' over the superior semicircular canal of the inner ear (indicated by the two white arrows).
Radiation Therapy
Treatment depends on the size of the tumor and the age of the patient. Periodic monitoring every 6-12 months may be the only necessary action, as 45% of tumors have little to no growth over a 3-5 year monitoring period. Radiation therapy, often in the form of gamma-knife radiosurgery, may be performed, or, in certain cases, surgical removal of the tumor is necessary.

Treatment of bilateral vestibular loss involved a special type of Physical Therapy known as Vestibular Rehabilitation Therapy (VRT). The type of VRT employed for bilateral vestibular hypofunction is quite different from the VRT used for unilateral vestibular hypofunction and improvement for those with bilateral vestibular loss is typically more slow and incomplete compared with unilateral vestibular loss. Patients with bilateral vestibular loss should avoid taking any medications that might reduce vestibular system function, including meclizine (Antivert), diazepam (Valium) or other benzodiazepine medications, and promethazine (Phenergan).
