Trauma to the foot and ankle region is extremely common. You don’t have to be extremely active to experience painful sprains, fractures and tears. Women over the age of 30 and men between 15- to 24-years old are the most susceptible to ankle sprains. Around 50 percent of sprains occur during physical exercise or athletics. Approximately 25,000 people in the United States sprain their ankle every day, leading to over 1 million emergency room visits per year.

Minimally Invasive Surgery
No. As in traditional surgery, there are some conditions that are contraindicated. Only an expert in minimally invasive surgery can decide what procedure is indicated. The vast majority of foot pathology can be corrected using MIS.
Ankle Surgery
Dr. Stephen A. Isham was a practitioner in Spokane, Washington for many years, and then at the Coeur d’Alene Foot & Ankle Surgery Center from 1995 until his retirement in 2010. Dr. Isham devoted his career not only to practicing these techniques, but teaching MIS procedures at home and abroad. His influence is prominent in many Central American, South American, European and Asian countries. He has pioneered and published articles on many of the minimally invasive surgical procedures currently in use, and has received numerous honors worldwide for his work in this field.
Bunion Surgery
Having spent at least three months researching minimally invasive bunion surgery before deciding to go ahead with it at CDFA, I feel that any helpful information put out there for potential patients like myself is of immeasurable value.
Bunions
Conservative treatments for bunions are often limited because they cannot correct the bone deformities. There is no way to stop the progression or reverse the deformity without moving the bones back into the correct position and realigning the joint. This can only be accomplished with surgery. When conservative treatment fails or the bunion progresses to the point where conservative treatment is no longer a viable option, surgical intervention may be needed to correct the deformity.
Toe Surgery
We offer multiple options for toe surgery including: aesthetic toe-shortening surgery, toe-straightening surgery, toe-lengthening surgery, and hammertoe surgery. All of our surgeries are performed via a ‘minimally-invasive approach’ which can be performed through a tiny key-hole incision.
Ingrown Toenails
There are three stages of ingrown toenails. In the first stage, the skin on either side of the nail is red and painful to the touch because of irritation, not infection. In the second stage, the skin is infected and may bulge over the side of the nail which may ooze clear fluid or pus. In the third stage, the skin has been infected for a prolonged time and is trying to heal itself.

As a result, the nail suffers constant damage and often becomes darkened and thickened from possible fungal infection or micro-trauma. The joints on the dorsum (top) of the toe typically rub and become irritated from the top of the shoe, causing unsightly corns, blisters, scarring, pain and occasionally open wounds. There is no practical or long term method to manage this problem without surgery, especially if shoe fashion is a priority.

No. It is absolutely false that minimally invasive surgery can be performed with laser. These procedures are performed through very small incisions using the latest and modern technologies resulting in excellent postoperative results. Lasers are reserved for soft tissue lesions such as warts, etc.

At your first appointment, we administer scanning to determine the precise point of trauma for optimal treatment. If a fracture is detected, we’ll discuss both surgical and nonsurgical rehabilitation methods (cast). The need for surgery mostly depends on whether your ankle is stable enough and bones are still in place. If not, we will use surgical methods to place a metal plate and screws to hold the bones together for ideal restoration. After the operation, patients are required to wear a splint and/or a cast. Bones typically heal in about six weeks, but ligament and tendon damage will take longer to recuperate. Keeping weight off the injured ankle is necessary for proper healing and re-positioning. Once mended, a few months of physical therapy helps patients regain normal function.

Excessive supination – This is the opposite of pronation and refers to the outward roll of the foot during normal motion. A natural amount of supination is also required. However, excessive supination places a large strain on the muscles and tendons that stabilize the ankle, and can lead to the ankle rolling completely over, resulting in an ankle sprain or total ligament rupture.

Ankle Sprain
Ankle injuries are caused by damage to the bone, ligament or tendon where three structural bones intersect: the tibia and fibula (lower leg) and talus (foot). Ligaments within the ankle joint keep these bones connected with strong elastic bands of tissue that provide a range of motion and support. Meanwhile, tendons attach muscles to the bones to help stabilize the joints and allow your ankle and foot to move on command.

A hammertoe is a contracture or bending of one or both joints of the second, third, fourth, or fifth toes. The contracture of the toes is a result of a muscle / tendon imbalance between the tendons on the top and the tendons on the bottom of the toe. As a result of the deformity the toe resembles a hammer.
Heel Spurs
On the bottom of the heel for release of the fascia and removal of the spur if present. There is no need for a long open incision thus causing minimal injury to the adjacent tissues which results in reduced swelling, pain, and recovery time. MIS surgeons are able to rely on a compression dressing for stabilization immediately after surgery, eliminating the need for internal fixation (pins, screws plates, etc.) The most commonly performed MIS procedure for correction of heel spur syndrome is the Isham modified Schwartz procedure.

Ingrown Toenails
Ingrown toenails (or onychocryptosis) occur when the edge of the nail grows into the skin of the toe. Ingrown toenails can be painful with redness and swelling around the nail.
Metatarsalgia
Metatarsalgia is a general term used to refer to a painful foot condition in the metatarsal region of the foot (the area between the arch and toes, or ball of the foot). The ball of the foot is a weight bearing area where the metatarsals bear the greatest amount of weight and pressure. If one or more of these metatarsals is out of alignment, then excessive pressure is generated in the area causing pain, inflammation and sometimes producing a callus. Many people say the painful feeling is “like walking on rocks”
Plantar Fasciitis
Heel pain is most often caused by plantar fasciitis, a condition that is sometimes also called heel spur syndrome when a spur is present. Plantar fasciitis is an inflammation of the thick band of fibrous connective tissue (fascia) running along the bottom (plantar surface) of the foot, from the heel to the ball of the foot. This strong and tight tissue contributes to maintaining the arch of the foot. It is also one of the major transmitters of weight across the foot as you walk or run. Therefore, the stress placed on this tissue is tremendous. This condition is one of the most common complaints relating to the heel. The pain is usually felt on the underside of the heel and is often most intense with the first steps of the day, or first step after a long period of being off your feet.
