Medical Weight Loss
From the standpoint of integrative and holistic medicine, one must ask the question: Do lifestyle changes work for improving ED or sexuality? There is increasing evidence that ED is associated with vascular disease. Risk factors such as diabetes mellitus, dyslipidemia, hypertension and smoking all present oxidative stress to the endothelial cells which form lining of the blood vessels. Endothelial cell injury and decrease in nitric oxide (NO) synthesis lead to impaired smooth muscle relaxation in the corpora cavernosa and ED. That being said, weight loss, management of metabolic syndrome/diabetes mellitus, control of blood sugar, blood pressure and cholesterol may improve sexual performance. There are several studies showing exactly what is predicted.

Fibromyalgia has been written by many health practitioners but it is worth revisiting from the acupuncture perspective. To me, FMS is a catch-all term for those who have widespread musculo-skeletal pain longer than 3 months, involving all four quadrants of the body, and a tender point count of 11 out of 18 points. Many patients with FMS also suffer from irritable bowel syndrome, chronic fatigue, mood disorder such as depression and anxiety, insomnia etc. According to the American Rheumatology Association, the etiology is not clear.

A young male engineer came to my office recently with a two-month history of diffuse dull abdominal pain. He had been constipated for a long time. Upon a recent visit to a local emergency room, an abdominal CAT scan was obtained showing significant amount retained fecal material. He had been a vegan for four years. On examination, his hands were very cold to touch, abdomen soft, but pulse deep, thin and week. Blood pressure was low, but pulse rate slow. He had the so-called “Kidney Yang Energy Deficiency” in Chinese medicine.

Kidney Stones
Kidney stone (nephrolithiasis) is a common problem in western countries, whose formation maybe related to high dietary calcium and protein. While many may pass their stones without intervention, the process is painful and unpleasant (referred to as renal colic.) Some may require further treatment, including insertion of a stent in the ureter, and lithotripsy (acoustic shock wave to break the stones) if stones are too large and too high.
Ovarian Cancer
Dr. Anil Sood and his colleagues at M.D. Anderson Hospital performed a retrospective review which showed women with ovarian cancer who also used beta-blockers such as Inderal (Propronolol) lived much longer than those who did not. Their finding was certainly not conclusive as we do not know many details about those women, such as body mass index, outcome of initial cytoreductive surgery, duration of beta-blocker usage, side effects of medication, the type of beta-blocker, etc. More research is needed in this topic.

Last if not least, it is estimated 10% of men with ED have very low blood testosterone level and these men benefit from testosterone replacement therapy, either transdermal in the form of gel or cream, or a long acting injection. Those on testosterone should undergo close monitoring because of potential side effects, especially risk of prostate cancer. For the majority of aging men with slightly decreased testosterone blood level, hormone replacement is a bit controversial at the present time. Some experts have encouraged the government to sponsor a large randomized clinical trial to fully address its risk and benefit.

Neuropathy is a common, frequently debilitating complication of cancer and its treatments. Unfortunately, there is no simple solution to this problem. It is possible that these patients represent a heterogeneous population and the underlying mechanisms of neuropathy may vary from individual to individual. Many confounding factors exist, such as presence of diabetes mellitus or other circulatory disease. The type of malignancy may be a factor also, some cancer such as myeloma may cause nerve damage itself. Dose, type and duration of chemotherapy also play an important role in prognosis. Personally, I believe that host factors should be taken into consideration too, such as nutritional status and degree of oxidative stress, that is, whether someone has adequate tissue levels of antioxidants for neuroprotection.
