My doughter have been seen at ...

My doughter have been seen at this office. the staff was very rude . they cancelt my app without notifieing me and gave me an app more than a month later . stating dr bonjoivanni could not see more than 4 patients because she doesnt kno how to use the cumputer system . when i called to seee if they could run my doughters request through the insurance again pam in billing told me to do it myself. her exect words were (why would i do tht? thts just more work for me) i asked her to repeat this because i didnt think i heord this right. and she repeated it. they are unproffetional rude and just downright none caring nore helpfull. never would i recomment this place . i wellcome a good doctor patient relationship. these ppl should not be allowed to practice in such an unproffetional rude manner. very disappointed very disturbing.

Irritable Bowel Syndrome
SBBO may cause or contribute to Irritable Bowel Syndrome (IBS) in some patients. Many of the symptoms of small bowel bacterial overgrowth are consistent with those of irritable bowel syndrome. An interesting observation is that there are a variety of intestinal bacteria that can predominate in patients with SIBBO. Different bacterial strains may produce different byproducts of their metabolism each of which can elicit different symptoms. For example, some bacteria produce methane, which can slow colonic motion and lead to constipation. Also, the quantity of bacterial overgrowth can wax and wane with time. This variation of bacterial types and quantity in patients may account for the fluctuating and alternating symptoms often seen in IBS.

Although there are many conditions that are not unique to women (ie. constipation, diarrhea, GERD/reflux, anemia), certain risk factors, as well as the course of illness, diagnosis, and treatment recommendations may be different between the sexes. Additionally, women may suffer from disorders affected by their anatomy, hormone fluctuations, pregnancy, and prior surgeries. For instance, women are more susceptible to pelvic floor disorders, including fecal incontinence, due to prior pregnancies and childbirth.

First-degree relatives (a parent, brother, sister, or child) of a person who has been diagnosed with an adenomatous polyp (or colorectal cancer) before the age of 60 years have an increased risk of developing adenomatous polyps and colorectal cancer compared to the general population. Thus, family should be made aware if the person is diagnosed with an adenoma or colon cancer.
Endoscopy
Gastroesophageal reflux occurs when abnormal amounts of gastric contents come up (reflux) into the esophagus. This is usually perceived as heartburn, chest discomfort, or a sense of regurgitation. Some patients have atypical symptoms leading to problems outside of the esophagus, per se. These might manifest as chronic cough, chronic sore throat or throat clearing or hoarse voice. Reflux has also been associated with poor sleeping pattern. After delving into the patients symptoms and performing a pertinent physical exam, testing may be recommended. Endoscopy (EGD) allows direct visualization of the esophagus and upper GI tract to inspect for reflux and related inflammation, assess for the presence of Barrettâ€™s (see below) and other complications of chronic reflux, examine the anatomy for significant hiatal hernia or other abnormalities, and to see if the stomach is functioning properly. Esophageal acid analysis by measuring pH in the esophagus over 1-2 days can objectively quantify reflux. This can be done as part of an endoscopy by attaching a small battery powered wireless pH sensor in the esophagus that measures acid reflux for 48 hours then later spontaneously falls off and is harmlessly passed. Esophageal pH can also be measured with a nasal catheter ambulatory 24 hour pH probe combined with impedance testing. Esophageal impedance is a newer technology that we introduced into the community via leadership as the Director of the Swallowing Center at The Christ Hospital. This allows the addition of assessing esophageal symptoms that may be related to reflux of nonacidic gastric contents. After evaluations, an individualized treatment plan is then recommended including dietary and life style advice, medication, and occasionally surgery.

Endoscopic ultrasound involved inserting a special scope that has an ultrasound probe on it into the rectum. In rectal cancer, this can accurately assess the size of the tumor and look for local spread outside the wall of the rectum and lymph nodes.
MRI
To evaluate the lower intestine,Â colonoscopy is usually the test of choice. Colonoscopy allows for comfortable direct inspection of the colon and the last portion of the small intestine (terminal ileum) using a fiberoptic scope in a sedated patient. Crohnâ€™s Disease appears as redness, bleeding, ulcers or sores in a spotty distribution. Ulcerative colitis appears as confluent redness and bleeding. An x-ray examination of the small bowel ( small bowel follow through or enteroclysis ) evaluates the small intestine for active Crohnâ€™s Disease or stricture (narrowing) or fistula. This is performed by drinking barium while taking abdominal x-rays as the barium flows down the intestinal tract. AÂ CT Scan Â is an x-ray technique allowing for cross-sectional imaging of the abdominal structures. This can help to identify areas of bowel wall thickening/inflammation and also look for pockets of infection (abscess).Â MRIÂ uses magnetic forces to image the body and does not involve radiation.Â Both CT and MRI can evaluate the small bowel (CT-enterography and MR-enterography). Endoscopic ultrasoundÂ utilizes an ultrasound probe placed under endoscopic control to carefully evaluate the area around the rectum for infection and inflammation, especially in Crohnâ€™s disease.Â Â Blood Â work Â can evaluate for anemia (low blood count) and evaluate the liver and other parameters. Blood levels of inflammatory markers including the white blood cell count, C-reactive protein (CRP), and erythrocyte sedimentation rate (ESR or sed rate) can signify inflammation or infection. Blood testing for certain IBD-related markers can sometimes support a diagnosis in some patients.

Complicated diverticulitis â€“ An abscess would be drained under CT scan guidance. Surgery would be needed for blockage or peritonitis or fistula. Bleeding from diverticula is usually managed medically (blood transfusions etc.) as the bleeding will stop on its own 80% of the time. Surgery for bleeding is done if bleeding continues or recurs significantly.
Chemotherapy
Stage IV cancer is called advanced colorectal cancer and is generally treated with chemotherapy; some patients may benefit from surgery.
Radiation Therapy
Earlier stages of disease (stages I through III) are referred to as localized colorectal cancers and generally treated with surgery, with or without chemotherapy. When the cancer is in the rectum, radiation therapy may be used.

Hysterectomy
Colon cancer screening is appropriate for all persons, regardless of gender, beginning at age 50 for those at average risk. However, there are some women in whom this would be recommended earlier, such as those with some gynecologic cancers. Colonoscopies themselves may be more difficult in women due to a more compact body habitus, slightly longer colon, and prior pelvic surgeries, like Caesarean section and hysterectomy. We understand these differences.

Esophageal motility disorders cause abnormalities in the contraction mechanism of the esophagus usually manifesting as non-cardiac chest pain or the sensation that food and/or liquids seem to get hung up in the chest after swallowing. The primary motility disorders are achalasia, diffuse esophageal spasm, nutcracker esophagus, and ineffective peristalsis. Esophageal function is usually assessed with motility testing, also called manometry. We now offer High Resolution Manometry that more accurately evaluates the motor function of the esophagus. Patients with esophageal motility disorders are then treated, for example in achalasia, they may undergo pneumatic endoscopic balloon dilation, BoTox injection, or be referred to a surgeon, as appropriate for each individual circumstance based on detailed consultation.
