Cosmetic Surgery

Medically necessary services shall not include: (a) non-medical services, such as social, educational, and vocational services; (b) cosmetic surgery; canceled or missed appointments; (c) research or the provision of experimental or unproven procedures; (d) the provision of whole blood; provided, however, that administrative and processing costs associated with the provision of blood and its derivatives shall be payable; and (e) private room differential.
