Repair of fecal incontinence stemming for example from an anal sphincter tear sustained in childbirth should ideally occur prior to menopause. The typical patient who sustains an anal sphincter injury at the time of a vaginal delivery might be advised to consider repairing the torn sphincter after childbearing is over, but prior to menopause, as muscle mass begins to atrophy in women over the age of 50, thus making repair of anal sphincter injuries more difficult.

Ovarian Cancer
Hysterectomy refers to surgical removal of the uterus. In most cases, the cervix is removed as well (total hysterectomy). Common indications for hysterectomy include symptomatic fibroids, abnormal uterine bleeding, pelvic pain, and endometrial cancer. A hysterectomy may also be performed at the time of a surgery for pelvic organ prolapse. Dr. Margolis generally recommends removal of the fallopian tubes at the time of hysterectomy. A growing body of literature suggests that removing the fallopian tubes decreases the risk of ovarian cancer, which in some cases arises from the fallopian tubes.

Hysterectomy
Although the goal is always to try conservative therapy where appropriate, many women will eventually require definitive surgical management via removal of the uterus (hysterectomy). Patients will benefit from Dr. Margolis’ decades of experience in both open and minimally invasive surgical techniques, especially in more challenging cases where patients have prior abdominal surgeries, enlarged uterus, or other significant medical conditions.
Laparoscopy
Conservative surgery is typically carried out by laparoscopy. Endometrial implants may be excised or obliterated. If the disease is extensive and the anatomy is distorted, laparotomy (opening of the abdominal wall via a larger incision) may be required. While surgical treatments can be very effective in the reduction of pain, the recurrence rate of endometriosis following surgical treatment can be high, so oftentimes women are advised to take oral or injectable medications after surgery to help suppress endometriosis.
Reconstructive Surgery
In addition to the significant complications associated with vaginally placed polypropylene mesh for prolapse, mesh has also been used abdominally/laparoscopically/robotically for pelvic organ prolapse with similar and in some cases more catastrophic complications. Published literature and Dr. Margolis’ experience has shown that polypropylene mesh is unsuitable for any pelvic reconstructive surgery.

Physical Therapy
Treatment options begin with conservative measures such as behavioral adjustment and avoiding dietary triggers. Avoiding caffeinated beverages and drinking 60-80 ounces of water daily are good places to start. Additional options include Kegel exercises/physical therapy, medications, pessary support devices or surgical management if indicated. Depending on the nature of the incontinence and medical co-morbidities, surgical options may include laparoscopic retropubic urethropexy (laparoscopic Burch), or organic slings.
